The urinary symptoms are most important. Useful in renal colic, prostatic affections, and catarrh of bladder. Sensation as if bladder were distended, with pain. Pain going down thigh.

Urinary

Black, bloody, thick mucous urine. Constant urging; great straining; pain down thighs during efforts to urinate. Can emit urine only when he goes on his knees, pressing head firmly against the floor. Feeling of the bladder being distended and neuralgic pain in the anterior crural region. (Staph.) Dribbling after micturition. (Selen.) Violent pain in glans penis. Itching along urethra; urethritis, with prostatic trouble. Inflammation of urethra; becomes almost cartilaginous.

Urinary Organs

Micturition difficult, with much pressing and straining only in drops, with the sensation as if the urine should be emitted in large quantities. Violent pains in the bladder, and at times in the back ; the left testicle is painfully drawn up ; pain in the thighs, shooting down into the toes and soles of the feet. Paroxysms of violent pain with the strangury ; he cries on loud, and can only emit urine when he goes on his knees pressing his head firmly against the floor ; remaining in this position for ten to twenty minutes, perspiration breaks out, and finally the urine begins to drop off with interruptions, accompanied by tearing, burning pains at the point of the penis. Urine smells strongly of Ammonium, and contains a large quantity of viscid, thick, white mucus. The paroxysms appear generally from 3 to 6 A.M. ; amelioration through the day. Almost cartilaginous induration of the mucous membrane of the bladder. Similar conditions of the bladder are to be found under Berberis vulgaris, but not the ammoniacal smell ; and the pain under Berberis is in the hip, not, as under Pareira, in the thigh. Berberis has also paralysis of the bladder with discharge of great quantities of mucus, which deposits itself at the bottom of the vessel and is viscid.

Characteristics

Pareira is a climbing plant, a native of the West Indies, Central America, and India. The root has a traditional use in diseases of the bladder and urinary organs. A short proving by C. M. Fox, of Denver ( Med. Adv ., xvi. 319), brought out some characteristic symptoms which have been clinically confirmed. Among these are: Violent pains with strangury; must go on "allfours" and press head against floor to pass water at all." "Urethritis with severe pain when passing water, and discharge of mucus from urethra." "Pains down thighs and even to feet when attempting to pass water." "Violent pain in glans penis" with the straining. Fox relates two cases of prostatic enlargement with dysuria in which Par . gave complete relief. ( 1 ) Man, 68, old prostatic affection constant urging with entire inability. Feet and legs much swollen had to be put in a warm bath before catheter could be passed. Only on hands and knees, with forehead pressed on the floor, could he pass a few drops of scalding urine. Par. b . 1x gave rapid relief. Swelling disappeared from legs, and he was able to resume his business and continue at it for years. ( 2 ) Enlarged prostate with retention; severe attacks two or three times a year, when he would have to have the urine drawn off for a week or two. After using Par. b . the catheter was not required more than twice in two years. When he feels the symptoms coming on a few drops of Par. b . relieve him at once. Many other remedies had been used in vain before Par. b . was prescribed.

Enlargement of prostate gland with retention of urine ; two or three times a year patient is seized with severe and painful attacks, during which catheter had to be used for several weeks.
Severe pains at times in back and bladder with painful retraction of left testicle ; pain in thigh extending down to big toe and sole of foot ; frequently an irritable eruption on legs and great irritation on scalp ; urine passed with great difficulty ; feels a great desire to urinate, fancies quarts will come away, but finds the greatest difficulty to get any to pass, notwithstanding the severe pressive pains attending it ; pains so severe that he is obliged to get on knees, press head firmly against something and remain in that position, forcing until he sweats profusely and "roars like a bull," to use his own expression ; after remaining in this situation ten or twenty minutes, urine dribbles away with great pain and frequent stoppages, scalding and lacerating at point of penis ; urine has strong ammoniacal smell, and is loaded with thick, tenacious mucus ; paroxysms generally occur from 3 to 6 A. M. ; during day comparatively easy ; cartilaginous condition of inner coat of bladder which felt very hard to the sound ; sweats at night ; hammering and buzzing noises in head. θ Chronic catarrh of bladder.
