Keynote

Diphtheritic and glandular affections of left side; fauces dark red; solids or liquids painful when swallowing; exudation slight, easily detached; cases attending epidemic scarlet fever, ulcers on fauces or tonsils; glands enlarged; greenish tough lumps from pharynx or posterior nares. Tubercular pharyngitis. Mercurius Solubilis. Hahnemann's Soluble Mercury. Nervous affections after suppressed discharges especially in psoric patients (Asaf.). Glandular and scrofulous affections of children. Otorrhoea: bloody, offensive discharge, with stabbing tearing pain; < right side, at night and lying on affected side. Furuncles and boils in external meatus (Pic. ac.). Polypi and fungous excresences in external meatus (Mar. v., Thuja). Acrid nasal secretion, having odor of old cheese; nostrils, red, raw, ulcerated. Epistaxis: when coughing; at night during sleep; hangs in a dark clotted string from the nose, like an circle. Gonorrhoea: with phymosis or chancroids; green discharge, < at night; urging to urinate; intolerable burning in for part of urethra when passing last few drops; prepuce hot, swollen, oedematous and sensitive to touch; of a torpid character, with threatening or suppurating bubo. Chancre: primary; regular indurated Hunterian, with lardaceous base; with cheesy bottom and inverted red edges; with phymosis or paraphymosis; deep, round, penetrating, eating through fraenum and prepuce; bleeding, painful, yellowish fetid discharge. Hahnemann's remedy for syphilis and diseases of the genito-urinary tract. Is rarely indicated if the tongue is dry. Disease of the skin; intolerable biting, itching, over body, as from insect bites, < in evening and from warmth of bed; becomes pleasant on scratching. Weakness and weariness of limbs; sore, bruised.

Common name

Bin-iodide of Mercury

General

Diphtheria and ulcerated sore throats, especially on left side, with much glandular swelling. Chronic suppurating buboes. Hard chancres. Old cases of syphilis in scrofulous patients. Early stages of cold, especially in children.

Throat

Fauces dark red; swallowing painful. Phlegm in nose and throat. Disposition to hawk, with sensation of a lump in throat. Stiffness of muscles of throat and neck.

Diphtheria; submaxillary glands painfully engorged, fauces dark red; worse on left tonsil. Parenchymatous tonsillitis. Will often abort peritonsillitis if given frequently. Cough form elongated uvula, with sore throat, Laryngeal troubles with aphonia.

Nose

Coryza and dull hearing; right side of nose hot. Hawks mucus from posterior nares. Turbinated bones swollen. Boggy mucus membrane of nose and throat; closure of Eustachian tube, opening with a pop.

Mouth

Gums swollen; toothache; glands swollen. Scalded feeling on tongue. Aphthæ. Profuse saliva. Tongue feels stiff at base, and pains on moving.

Skin

Small fissures and cracks; hard papules; Hunterian chancre; syphilitic ulcers. Bubo. Sarcocele.

Dose

Third trituration. Mercuric iodide is far more active as a bactericide than the other mercurials, including the chloride.

Characteristics

Merc. bin . was introduced and proved by the American Provers' Union in 1856, and is an important addition to the mercurial group. It acts more intensely on the lymphatic glands and cellular tissue than Merc. v ., which it otherwise closely resembles. It has been extensively tested in throat affections. It acts more on the left side, or primarily on the left. The symptoms are < on empty swallowing; also < on swallowing food. Cases have been recorded in which small doses of Merc. bin . 3x or 2x have cut short an attack of asthma; or, if taken at bed-time, have prevented an expected attack from developing during the night. The explanation proffered is that Mercury , and especially its iodides, render the blood incapable of holding in solution large quantities of uric acid and urates, which are presumably the irritant factor in an asthmatic attack. But the pathogenesis of Merc. bin . has oppressed breathing, and if the general Merc . conditions are present it is quite capable of acting dynamically. ─ Headache is < in open air. Eyes < by bright light; psoric eye affections. Right side of nose is inflamed. The liver, spleen, and pancreas are affected; and also the bladder and male sexual organs. Wandering rheumatism. Troublesome dreams. It acts well in old cases of syphilis, particularly in persons of lax fibre, scrofulous, and those who have taken much Mercury . The symptoms are < after sleep. < After dinner. < Afternoon and evening. Open air > head. Getting warm walking > deafness. Cold air, cold washing, getting wet < . Touch and pressure < .

Causation

Wetting. Washing floor. Weather changes.

Mind

The head is worse; but he is good-humoured, even merry. ─ Low-spirited; disposed to cry. ─ Ill-humoured and bad taste on waking in morning.

Head

Vertigo; everything seems to reel round her. ─ Sensation as if bound by a tight cord in frontal region. ─ Dulness in head and slight pressing pain in l. side, as during coryza; > walking in open air. ─ Pain in head with heat. ─ Great heat in vertex with pulsation, 11 p.m. ─ Small pustules on head. ─ Pain in bones of head, chiefly occipital. ─ Syphilitic tumour in brain.

Eyes

Inflammation, with burning, watering; < bright light. ─ Albuginea inflamed and painful. ─ Psorophthalmia; trachoma; granular lids; pannus.

Ears

Hearing dull; > evenings; ears close for a few moments at a time; coryza. ─ Earache (r.). ─ Wax increased. ─ Itching in both ears. ─ Swelling of parotid and neighbouring glands.

Nose

Coryza: with headache; dull, hearing, > getting warm walking; r. side nose hot, swollen; much sneezing and running from head; hoarseness. ─ Whitish yellow or bloody discharge; affection of posterior nares, with raw sensation; nasal bones diseased; turbinated bones swollen. ─ Hawks mucus from posterior nares. ─ Crusty eruption on wings of nose. ─ (Polypus l. side.)

Face

Pressive sticking; heavy aching in upper jaw. ─ Aching pain in l. cheek and l. eye, which seems slightly inflamed. ─ Parotitis (r.). ─ Lips slimy and sticky on waking. ─ Eczema rubrum on chin.

Teeth

Fine stinging in teeth; painful jerks. ─ Toothache < by chewing; < after eating; with ill-humour. ─ Gums swollen, excessively tender.

Mouth

Profuse flow of saliva, and aching in teeth of lower jaw. ─ Taste: bitter; slimy; metallic. ─ Mucous membrane inflamed with burning. ─ Mucous patches. ─ Nodules. ─ Boils in mouth.

Throat

Much phlegm in throat and nose; hawks it out. ─ Sensation of a lump in the throat, with disposition to hawk it out; hawks up a hard, greenish lump. ─ Throat feels as if scalded on waking. ─ Inflammation and swelling of l. tonsil, velum elongated causing cough, next day both tonsils involved (l. to r.). ─ Painful swelling of tonsils and submaxillary glands. ─ Fauces dark red. ─ Diphtheritic patches, and superficial ulcers in throat. ─ Diphtheria r. side < from empty swallowing; throat sensitive to touch. ─ Tubercular sore throat.

Appetite

Thirst for small quantities. ─ Wants food more salted.

Stomach

Heartburn after dinner. ─ Loud and bitter belching. ─ Sinking. ─ Pain in epigastrium on pressure.

Abdomen

Aching and fulness in r. hypochondrium. ─ Sudden cutting in liver. ─ Heavy pain in liver, pancreas and spleen. ─ Transient drawing pain followed by lameness in l. hypochondrium; l. waist feels sore on bending. ─ Enlarged malarial spleens and livers. ─ Abdomen distended at navel; sore to pressure. ─ Colic; uneasy sore feeling. ─ Buboes.

Stool and Anus

Stool: copious, yellow brown, and somewhat watery, and coated with mucus and slightly bloody; preceded by colic, urging and slight tenesmus remained. ─ Pain in anus as if piles would appear. ─ Inveterate piles.

Urinary Organs

Bright's disease. ─ Ulcers in bladder. ─ Frequent urgent desire. ─ Increased flow of urine; thick and dark when passing; red.

Male Sexual Organs

Desire, esp. on going to sleep; emissions. ─ Sensitiveness of r. testicle and cord. ─ Sharp shooting stitches in end of penis through glans. ─ Gonorrhœa; discharge free and mucoid; patches of induration, along urethra. ─ Hard, red swelling in front of prepuce, and painless hard chancre in centre. ─ Sarcocele of l. testicle; syphilis. ─ Bubo, discharging for years; indolent chancre.

Female Sexual Organs

Yellowish leucorrhœa. ─ Stony hardness of fibroid tumour. ─ Great soreness in whole breast.

Respiratory Organs

Cough: from elongated uvula; with sore throat; with a little loose, whitish, slimy sputum. ─ Profuse yellow sputum. ─ Hoarse and husky, after getting a little wet in a shower.

Chest

Constriction across chest. ─ Catching pain under r. breast, oppressing breathing. ─ Swelling of bronchial glands from atmospheric changes.

Heart

Sticking pain in heart.

Neck and Back

Swollen neck glands, with toothache, scarlatina, &c. ─ Spine sore and painful.

Limbs

Rheumatic pains, now here, now there, mostly muscular.

Upper Limbs

Rheumatic pains in shoulder-joint. ─ Axillary glands suppurate. ─ Strained feeling in middle of humerus as if about to break. ─ Palm of l. hand cracked, horny, several oozing rhagades.

Lower Limbs

Aching in bones from hips to ankles. ─ Pains from calves up to sacrum. ─ Unbearable pains in legs towards evening, > on moving. ─ Pain of syphilitic ulceration in front of leg.

Skin

Pustules, with inflamed base, sore to touch; itching slightly, scab over, but pus oozes. ─ Small fissures and cracks. ─ Lupus. ─ Condylomata. ─ Freckles; liver spots. ─ Syphilitic eruptions and ulcers.

Sleep

Dreams: frightful; of travelling; swimming; lascivious. ─ Symptoms appear or are < on waking.

Fever

Intense shivering, then feverishness; copious sweat at night. ─ Fever with grippe.

Eye got sore in a single night, on following day much swollen, lids could not be turned out without much pain, being very sensitive to touch ; a stream of hot, acrid tears, mixed with yellowish flakes of fibrinous material gushed out ; marked chemosis of a pale yellowish color,
presenting a succulent appearance ; marked infiltration throughout entire conjunctival surface, presenting a firm fibrinous nature ; over both palpebral surfaces a thick, opaque membrane had formed, which could be detached in small patches and shreds ; much extravasation of
blood over entire ocular conjunctiva ; cornea invaded by ulceration at three distinct points, larger and deeper of these being at centre, threatened to penetrate into anterior chamber. (Arg. nit. was used externally). θ Diphtheritic
conjunctivitis.
Old cases of granular lids and pannus.

Hearing and ears

Hearing dull ; > evenings ; ears close for a few moments at a time ; coryza.
Earache in right ear.
Earwax increased.
Itching in both ears, all night.
Swelling of parotid and neighboring glands.

Smell and nose

Coryza : with headache ; dull hearing > getting warm by walking ; right side of nose hot, swollen ; much sneezing ; with hoarseness.
Whitish yellow or bloody discharge ; affection of posterior nares, with raw sensation ; nasal bones diseased ; turbinated bones swollen.
Hawks mucus from posterior nares.
Swelling ; hot and inflamed state of right side of nose, with coryza.
In left nostril a yellowish white tumor, spongy, moist, extending into orbit behind eye, displacing it one half its diameter to left external angular process of orbit, and protruding about one third of an inch further than normal. θ Nasal

Upper face

Aching in left cheek and eye.
Scabs on face, right side.

Lower face

Lips slimy and sticky on waking.
Lips stick together ; headache.

Teeth and gums

Gums swollen ; toothache ; glands swollen ; boil in mouth ; sleepless ; alternately melancholic and cheerful, in periodic spells.

Taste, speech, tongue

Taste : slimy on waking ; bitter, metallic.
Tongue : dry, wants to wet mouth ; scalded feeling, small blister on point ; furred, with grippe ; aphthæ.

Inner mouth

Profuse flow of saliva and aching pains in teeth of lower jaw.
Soreness of middle of inside of left cheek.
Mucous membrane of mouth inflamed, with burning.
Mucous patches on lips and inner surface of cheeks ; round spots, but slightly demarcated, bare of epithelium and covered with a creamy exudation ; discharge varies in character ; spots very sensitive, but do not bleed. θ Secondary
syphilis.
Small nodules on mucous membranes of mouth, pharynx and nose, which become round ulcers, with hard edges and lardaceous base ; slight pain ; soon disappear from original place and appear in another part ; when appearing on tongue, lip or wing of nose a distinct deep
induration can be felt surrounding them.

Palate and throat

Much phlegm in throat and nose.
Disposition to hawk ; sensation of a lump in throat ; hawked up a hard, greenish lump.
Hawks much ; spits a tough, white phlegm.
Sticking in throat.
On waking throat sore ; feels scalded, < during empty swallowing.
Loose cough, back of throat and nose inflamed, glands of throat ulcerated ; enlarged tonsils, breathes with mouth open and snores at night ; expectoration yellowish greenish, viscid or purulent.
Pharynx more irritable than larynx, with affection of posterior nares ; great sensitiveness to cold air. θ Sore throat.
Follicular catarrh of pharynx ; engorged tonsils.
Painful swelling of tonsils and submaxillary glands.
Left tonsil swollen ; fauces dark red ; diphtheritic patches ; submaxillary glands painfully engorged.
Slight superficial ulcers in throat in patches.
Difficult deglutition, with ulcers in throat.

Eating and drinking

Heartburn after dinner.

Hiccough, belching, nausea and vomiting

Loud and bitter belching.
Nausea : sore throat ; sinking at stomach and epigastrium, with general sick feeling ; while passing diarrheic stool.

Scrobiculum and stomach

Pain on pressure at epigastrium.

Hypochondria

Aching and full feeling in right hypochondrium.
Sudden cutting pain in region of liver.
Heavy, painful feeling in region of liver, pancreas and spleen.
Transient drawing pain, followed by lame sensation in left hypochondrium ; left waist feels sore on bending.
Enlarged malarial spleens and livers.

Abdomen and loins

Distended abdomen about navel, with pain on`pressure.
Colic, followed by stool.
Uneasy, sore feeling all over bowels.
Subacute gonorrhœal inflammation of inguinal glands ; swelling pear-shaped, not very hard nor painful, pain only present upon strong pressure.

Stools and rectum

Stool copious, yellow brown, somewhat watery, and coated with mucus and slightly bloody, preceded by griping and colicky pains ; urging and slight tenesmus remained afterwards.
Inveterate piles.

Urinary organs

Bright's disease.
Ulcers in bladder.
Increased flow of urine.
Frequent desire to urinate, she cannot hold her urine for a moment.
Urine : increased flow ; thick and dark when passing ; red.

Male sexual organs

Sexual desire, particularly on going to sleep ; nocturnal emissions.
Sensitiveness of right testicle and cord.
Left side of scrotum for many years slightly enlarged, accompanied by varicocele ; sudden, rapid increase of size, finally presenting a hard, painless tumor 19 cm. long and 9 cm. in circumference ; no hydrocele ; no inflammatory symptoms. θ Sarcocele.
Sharp shooting stitches in end of penis through glans.
Threatened gangrene of glans in paraphimosis.
Hard, red swelling of front of prepuce, appearing as thick and hard as lead pencil, with hard chancre in centre, entirely painless.
Discharge free and mucoid ; patches of induration along urethra. θ Gonorrhœa.
Bubo discharging for a year.

Female sexual organs

Leucorrhœa of yellowish color.
Stony hardness of fibroid tumor.

Voice and larynx. trachea and bronchia

Complete loss of voice. θ Diphtheria.
Hoarse and husky shortly after getting a little wet in an evening shower.
Patches of inflammation of a livid purplish hue, discharge thin and offensive. θ Laryngitis.
Dark red inflammation and swelling of parts, with much hawking, coughing and purulent expectoration, < in morning. θ Phthisis laryngis.
Swelling of bronchial glands ; subacute processes arising from influences of cold or atmospheric changes.
Goitre (Iodine was used externally).

Cough

Cough : from elongated uvula ; with sore throat ; with a little loose, whitish, slimy sputum.
Grippe, with fever, headache, giddiness, furred tongue ; sweat in bed.
Profuse yellow sputum.

Inner chest and lungs

Awoke from a transient feeling of soreness in whole breast.
Constriction across chest.
Catching pain under right breast oppressing breathing.

Heart, pulse and circulation

Sharp cutting pain in chest.
Sticking pain in heart.

Outer chest

Sticking in muscles of ribs, left side, after walking out during thawing weather.

Neck and back

Swollen glands on neck, with toothache, scarlatina, etc.
Spine sore and painful.

Upper limbs

Rheumatic pains in shoulder joint.

Dull, aching, strained sensation in middle of os humeri as if about to break.
Rheumatic pains, soreness, stiffness in left arm < by motion.

Rheumatic pains now here, now there, mostly muscular ; alternately in arms and hands, legs and feet ; violent pain like otalgia in left ear.

Nerves

Epileptic spasms increase two days and are gone the fourth.

Insomnia, with ulcerated throat.
Restless from 12 P. M. till morning, with constriction in diaphragm.
Dreams : frightful ; of swimming ; gunning ; traveling ; lascivious.
On awaking : ill humor ; dull head ; slimy taste ; throat sore ; pain in chest.

Morning : on awaking ; ill humor and bad taste ; coughing and purulent expectoration <.
After dinner : heartburn.

Aching : in left cheek and eye ; in teeth of lower jaw ; in right hypochondrium ; from hip to ankles ; in legs.
Slight pressing pain : on left side of head.
Heavy, painful feeling : in region of liver, pancreas and spleen.
Dull, aching, strained sensation : in middle of os humeri.
Burning : of mouth.
Soreness : of middle of left cheek ; of throat ; of waist ; all over bowels ; in whole breast ; of spine ; of left arm.
Raw sensation : of posterior nares.
Bruised feeling : in forearm.
Sensitiveness : of right testicle and cord.
Pressure : over eyes.
Full feeling : at epigastrium.
Dulness : of head.
Slight pulsation : in vertex.
Constriction : across chest ; in diaphragm.
Stiffness : of left arm.
Lame feeling : in left hypochondrium.
Sinking : at stomach.

Tissues

Indurated chancres ; Hunterian chancre.
Chancre and bubo, particularly indolent.
Old cases of syphilis, particularly in persons of lax fibre, scrofulous, and in those who have taken much mercury.

Touch. passive motion. injuries

Touch : eyelids very sensitive ; throat sensitive ; pustules sore.
Pressure : pain at epigastrium ; pain about navel ; pain of inguinal glands.

Skin

Small fissures and cracks.
Hard papules here and there.
Pustules, inflamed base, sore to touch, itching slightly ; scab over, but pus oozes.
Syphilitic ulcers.
Freckles, liver spots.
Chilblains.
Old scrofulous or syphilitic ulcers, spreading.
Syphilitic eruption over whole body, with moist, offensive smelling ulcers on legs, of two years' duration.
Lupus ; condylomata.
During course of malignant scarlatina swelling of parotid and neighboring glands ; fauces and tonsils covered with large fetid ulcers, and fever increased, with delirium, insomnia and difficult deglutition.

Stage of life, constitution

Girl, æt. 16, thin, weak ; diphtheria.
A servant girl, after washing floor ; rheumatic affection.
Man, æt. 29 ; suffering a year ; bubo.
Irish laborer, æt. 40 ; living in damp, ill ventilated house, of irregular habits, occasionally indulges in a drunk ; diphtheritic conjunctivitis.
Man. æt, 70 ; sarcocele.

Antidoted by : Hepar .
Compatible : after Bellad ., in scarlatina.
Compare : Badiaga, Carbo an . and Nitr. ac ., in syphilitic eruptions ; Merc. præc. rub ., which is preferable for leaden heaviness in occiput with otorrhœa.

Lecture

Again, if in a Merc. patient with diphtheria, tonsillitis, etc., the inflammation and pain begin on the left side, and incline to remain there or spread to the right, the bin-iodide is indicated.

These two iodides have more rapid and greater induration beneath ulcers and chancres than Merc., and in old syphilitics the iodides are sometimes more useful.
